About
We are a small team of clinicians who share a passion to improve the health outcomes of the local homeless population.
We do this by providing outreach clinics at the local homeless centre run by HARP and at two of the soup kitchens in town.
We have developed a collaborative model working with the Rough Sleepers Mental Health team, Forward Trust (Drug and Alcohol services), Tissue Viability team, Community Diabetic team and local GP practices.
As well as providing acute services (including prescribing and referrals to secondary care) we offer chronic disease management and health screening.
